秦皇島之名的起源地是指今海港區東山,這是一座由風化花崗岩組成的剝蝕性殘山,海拔20餘米,方圓不足平方公里。今天與陸地連在一起的東山,在200多年前,還是一座名副其實的島嶼,直到19世紀末,隨着港口的建設,島嶼才與陸地相連,成爲大陸的一部分。

秦皇島相傳因秦始皇東巡至此而得名。秦皇島之名最早見於明英宗天順五年(公元1461年)楊琚《秦皇島》一詩,其中有“古殿遠連雲縹緲,荒臺俯瞰水潺湲”之句。

到清代,又有秦王島之說。秦王島之名最早見於康熙八年《撫寧縣誌》,其中有“秦王島誤秦皇島,在縣東七十里,四面皆水,惟島居中,唐太宗徵高麗駐蹕於此,島上荊條伏生。相傳秦王下拜,伏。”
